Excursion Description:
As part of Year 10 & 11, students are invited to attend an excursion to Wet 'n' Wild during week 8 of term 4 - Friday 22/11/2024. This is to celebrate their hard work, improvements and accomplishments in 2024. They will be taken to Wet 'n' Wild via belted bus and supervised by their form teachers and year coordinators and returned by the end of the day. Students will adhere to College expectations which includes the students code of conduct.

Activity Risk Management Strategies:
Students will be supervised by College teachers during transport and at luch times. Wet 'n' Wild staff supervise all rides and swimming areas. First Aid available all day.
Dress Code:
Students are to arrive at school in their sports uniform. Students are required to wear appropriate swimwear; that is sun safe and has appropriate coverage of their bodies.
Students must have a hat, and will be provided with Sunscreen throughout the day.
Other:
Food is not provided by the school; however, students are welcome to bring their own food or money to access the venue food canteens.
Activity Costs:
$60.50 for transport and entry. If students already have an entry pass, please only pay for transport, which is $30. Both permission on Daymap and full apllicable payment is required by the due date for students to attend.
Privacy Notice:
The Department of Education and Training (DET) is collecting the personal information requested as part of this excursion process in order to:
- obtain lawful consent for your child to participate in the activity;
- help coordinate the activity;
- respond to any injury or medical condition that may arise during, or as a result of the activity; and
- update school records where necessary.
The information will only be accessed by authorised school staff and will be dealt with in accordance with the confidentiality requirements of as applicable, s.426 of the Education (General Provisions) Act 2006 (Qld) and the Information Privacy Act 2009 (Qld) and/or the Privacy Act 1988 (Commonwealth).
The information will not be disclosed to any other person or agency unless the disclosure is authorised or required by law, or you have given the Department permission for the information to be disclosed.
As excursions and camps are conducted away from school grounds, there is an increased duty of care, responsibility, and accountability placed on staff as there are fewer support resources available. Students who are deemed to have demonstrated regular, inappropriate, unacceptable and/or unsafe behaviours while in the capacity of a Capalaba State College student, may pose an unacceptable risk to the school's obligations under duty of care. Decisions about whether a student will attend an excursion or camp will be made by the Principal in consultation with the Deputy Principal and Year Level Coordinator after conducting a risk assessment in consultation with the parent of the student. The student and their family will be advised accordingly after a decision has been made. Students who are unable to attend an excursion or camp will have alternative arrangements made for a program of work to be undertaken at school for the duration of the activity.
Students will be expected to comply with the terms of the Student Code of Conduct (2020 - 2023) for the duration of this activity.
Activity Risks & Insurance:
Please note that the Department of Education and Training (DET) does not have personal accident insurance cover for students. If your child is injured as a result of an accident or incident while participating in the activity, all costs associated with the injury, including medical costs are the responsibility of the parents/carers. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.
The school collected medical information about your child at enrolment. This information is stored electronically in OneSchool. Please indicate by selecting the most appropriate answer below as to whether you need to provide us updated medical information relevant to the student attending the excursion as this may effect your child's full participation in the activity described in the form.
Acknowledging
- I have read all of the information contained in this form in relation to the activity (including any attached material);
- I am aware that the department does not have personal accident insurance cover for students;
- I give consent for my child/student to participate in the identified activity;
- I will pay to the school the costs detailed in this consent form for your student's participation in the activity;
- I agree to and understand the refund policy as it applies to this excursion (see Activity costs);
- In the event of an accident or illness, school staff may obtain or administer any medical assistance or treatment the child/student may reasonably require, including contacting their doctor;
- I accept liability for all reasonable costs incurred by the department in obtaining such medical assistance or treatment (including any transportation costs) and undertake to reimburse the department the full amount of those costs;
- I have provided the school with all relevant details of the child/student's medical or physical needs on registration/enrolment and where relevant have updated this information;
